What to Expect Before Mohs Cancer Surgery

What to Expect Before Mohs Cancer Surgery

What to Expect Before Mohs Cancer Surgery | Tampa | HillsboroughThe techniques for eliminating tumors have evolved rapidly since doctors initially started skin cancer treatment.

In Mohs surgery, a specially trained doctor eliminates one thin skin tissue layer at a time and then evaluates it under a microscope.

In case any cancer cells are seen, the surgeon eliminates another skin layer from the exact area where the cancer cells remain. The surgeon will repeat this process until no more cancer cells are detected.

Suncoast Skin Solutions, led by board certified dermatologist Dr. Christopher Ewanowski, provides cosmetic and medical skin care treatments to patients in Tampa, Brandon, Lutz, Seminole, Riverview, Brooksville, Winter Haven, Daytona Beach, Largo, St. Petersburg, Ocala, Sarasota, Central Florida, Florida, and surrounding locations.

The Preparation

After a patient has been scheduled for Mohs surgery, it is vital to take note of any existing medical conditions. For instance, if the patient has an artificial joint or a heart murmur, they should consult with their primary care doctor to understand whether pre-op antibiotics are necessary.

In case a patient has a pre-existing cardiovascular condition, it is important for them to speak to their doctor as well. They may or may not ask them to stop the use of blood thinners.

Patients without any pre-existing conditions will still have to keep a few things in mind. Specialists recommend avoiding aspirin and other nonsteroidal anti-inflammatory medications (including ibuprofen) for a minimum of seven days before the procedure.

Patients should also avoid vitamin E and alcohol for a few days before the surgical procedure. The patient should have a normal breakfast on the morning of the surgery as there is no need it fast since the procedure is done under local anesthesia.

It is advisable for the patient to clear their schedule on the day of the Mohs procedure as the process can take time. They may want to ask a friend or family member to accompany them to their appointment. Finally, if the procedure is being undertaken on the face, the patient should ensure that their skin is makeup free.

Tobacco and Blood Thinners

It will be necessary for the patient to avoid using tobacco in any form for a specified duration prior to the Mohs surgery. Tobacco smoke contains nicotine which can disrupt the smooth flow of oxygen in the blood. This can lead to an increase in surgical risk and the possibility of delayed healing.

In case the patient has been on blood thinners such as aspirin and NSAIDs or specific types of herbal supplements and vitamin E, they may need to discontinue their use for some time. The schedule of prescriptions drugs may also need to be altered.

Practical Arrangements

Working professionals should budget for a few days of leave from their workplace beforehand. Non-working patients should also plan to stay at home for some days and schedule their social engagements accordingly.

In addition, the patient should arrange for a responsible person to drive them home following the procedure. If required, they may arrange for aftercare for the initial few days of the recovery period.

The patient should ideally undertake any pressing home or office tasks beforehand to ensure a comfortable and relaxed recovery phase. They may also create a recovery kit at home containing the following items:

  • Prescription meds and antibiotics
  • OTC pain meds
  • Bandages
  • Loose fitting, front-open clothing
  • Favorite music CDs or movie DVDs

Why Choose a Board Certified Dermatologist?

A board certification in dermatology is awarded to only a handful of dermatologists each year. A board certification serves as an automatic assurance for the patient that the treatment provider has undergone a minimum number of years of dermatological training.

A board certified dermatologist will typically invest in top-notch quality standards and best practices, adopt safe and effective techniques, pursue continuing education and follow the most recent safety protocols to offer their patients the best possible results.

Qualities of an Ideal Dermatologist and Mohs Surgeon

A patient should seek the following qualities while choosing a dermatologist:

  • Caring, compassionate, warm, and easy to talk to
  • Inclined to inform the patient on all aspects of the procedure
  • Impassioned about making a positive difference in the lives of patients through dermatological procedures
  • Offers personalized care and attention to each patient

  • Possesses an in-depth understanding of various cosmetic and medical skin issues
  • Has a sense of proportion, balance, and form along with artistic sensibilities
  • Uses cutting-edge techniques to ensure that the procedure is minimally invasive and causes little to no scarring

Additionally, a dermatologist should be able to display before and after pictures of previous skin care procedure patients. They should also offer a new patient with testimonials from previous patients.

Individualized Care and Attention

The dependable Dr. Ewanowski offers a personal touch to the dermatologic care of each patient. He treats each patient as a member of his own family and is a unique combination of professional skills and knowledge along with the humane attributes of compassion and empathy.

The prudent Dr. Ewanowski will never adopt an “off the rack” or “one size fits all” approach to dermatology. He carefully assesses the patient’s condition, understands their concerns and personal objectives, and incorporates their desires to develop a customized dermatology treatment plan for best outcomes.

Expertise in Mohs Surgery

Mohs surgery refers to a subspecialty dedicated to the conservative removal of skin cancers while preserving as much tissue as possible and subsequently repairing the resulting wound aesthetically.

In comparison to routine excision, Mohs surgery looks 99 percent more at the edges of the eliminated tissue. Therefore, it offers a far higher clearance rate for tumors.

It can be troublesome enough to have surgery once for a basal cell on the nose or a melanoma on the cheek. But undergoing the process again for the same improperly excised cancer is unnecessary.

Mohs surgeons who undergo fellowship training spend an extra year of their training perfecting tumor extirpation, recognizing challenging histologic variants of cancers using the microscope, and refining grafts and flaps to yield less visible post-operative scars.

Finding The Right Mohs Surgeon in Pinellas County

Finding The Right Mohs Surgeon in Pinellas County | Tampa | St. PeteDr. Christopher Ewanowski has the distinction of being only one of a few board certified, fellowship-trained dermatologic surgeons in the Bay Area specializing in Mohs Micrographic Surgery, laser surgery, and reconstructive and cosmetic dermatologic surgery.

Dr. Ewanowski is a native of Tampa and attended Jesuit High School. After that, he was an undergraduate at The University of South Florida. Upon receiving his M.D. from the University of South Florida College of Medicine in 2003, he undertook a plastic and general surgical internship at the University of California San Francisco.

He focused on the cutting-edge surgical management of skin conditions including laser surgery, skin cancer surgery, and reconstructive surgery as a dermatology resident at Georgetown University Hospital.

After the completion of his training at Georgetown in 2007, he was only one of around 65 dermatologists globally to commence a dermatologic surgery fellowship focusing on skin oncology and Mohs, laser, and reconstructive surgery.

He completed this fellowship, which was approved by the American College of Mohs Micrographic Surgery in the year 2008.

Here Dr. Ewanowski trained under the guidance of Pearson Lang M.D. and Joel Cook M.D., two of the most renowned and talented Mohs surgeons in the country. After acquiring this wealth of knowledge and experience, he returned home and set-up Suncoast Skin Solutions in 2008.

Suncoast Skin Solutions, led by board certified dermatologist Dr. Christopher Ewanowski, provides cosmetic and medical skin care treatments to patients in Tampa, Brandon, Lutz, Seminole, Riverview, Brooksville, Winter Haven, Daytona Beach, Largo, St. Petersburg, Ocala, Sarasota, Central Florida, Florida, and surrounding locations.

Treatment Approach

The skin is the only organ that continuously replenishes itself, and this requires that the medical care providers evolve as well.

Suncoast Skin Solutions offers patients the most advanced treatments in all areas of dermatology. At the same time, the practitioners remain faithful to tried and true, and ‘old-school’ treatment approaches.

Expertise in Mohs Surgery

Mohs micrographic surgery is a very advanced and successful treatment for various types of skin cancer. The Mohs technique is thought to be the gold standard in the treatment of skin cancer. This procedure involves the surgical elimination of skin cancer in a layer by layer manner.

In this procedure, each layer is examined under a microscope until the cancerous tissue has been entirely removed and the margins are clear (the tissue around the tumor is free of cancer).

More traditional skin cancer treatment approaches estimate the extent of tissue to be treated. However, Mohs surgery examines 100 percent of the tumor margin enabling Dr. Ewanowski to take only the required amount of tissue to ensure that the cancer is completely eliminated.

This treatment approach minimizes the amount of normal tissue removed and enhances the aesthetic results.

Dr. Ewanowski offers general dermatology as well as surgical dermatology services. Patients experiencing a skin concern are encouraged to contact Dr. Ewanowski who specializes in advanced medical and surgical dermatologic services.

He provides advanced medical, surgical, and non-surgical treatments for various skin conditions and will be seeing general dermatology as well as surgical dermatology patients.

Patient-Centric Approach

Dr. Ewanowski’s patients all agree that he provides a personal touch to their medical care, which is a rarity in today’s world. He fosters a personal bond with his patients.

His motto, in fact, is to “always do the right thing.” Patients never have any doubts about his compassion, sincerity, and excellence. He constantly strives to be a leader in his domain and provides patients with the most superior options for their skin cancers and overall dermatologic requirements.

Types of Cosmetic Dermatology Procedures Available in Largo

Types of Cosmetic Dermatology Procedures Available in Largo | TampaCosmetic facial filler injections are currently the second most popular treatment in aesthetic dermatology in the US after Botox. Dr. Ewanowski and his team provide some of the latest FDA approved soft tissue fillers for facial volumizing.

Suncoast Skin Solutions, led by board certified dermatologist Dr. Christopher Ewanowski, provides advanced skin care treatments to patients in Tampa, Brandon, Lutz, Seminole, Riverview, Brooksville, Winter Haven, Daytona Beach, Largo, St. Petersburg, Ocala, Sarasota, Central Florida, Florida, and surrounding communities.

Dermal Fillers

Aging process can create a striking loss of volume in the face, causing deep furrows and wrinkles over the years.

These wrinkles can be eliminated in a non-surgical manner by filling them in and removing the depressed areas by using a type of injectable filler. This simple, in-office procedure involves less downtime while offering dramatic outcomes.

Hyaluronic acid (HA) derivatives are recent developments in dermal filling technology. These are well-tolerated and do not require prior allergy testing. Some brand name fillers in the HA family are Juvederm, Perlane, Restylane, and Captique.

There are various innovative products available to patients, such as Radiesse, comprising microspheres of calcium hydroxyl appetite suspended in an aqueous gel carrier, or Sculptra, a poly-L-lactic acid substance warranting multiple treatment sessions.

Restylane and other HA-based fillers maintain their shape through the body’s own moisture. The HA in Restylane is hydrophilic (“water-loving”). It soaks up and retains water to create a plumping effect beneath the skin.

HA slowly degrades with time, but each molecule binds to more water. Therefore, even with less HA, the same volume can be maintained over time. The ability of HA to bind to water is the reason for its sustainability.

Nasolabial folds and lips are the regions that respond effectively to treatment with injectable fillers such as Restylane. However, the choice of the right type of filler, the accurate quantity of filler, and the timing of the treatment are factors that separate excellent outcomes from average ones.

Every Situation is Different

Suncoast Skin Solutions offers various medical spa injectable treatment options, but will recommend one that is most suitable for a specific patient. The professionals at Suncoast do not believe in a “cookie-cutter” approach. It is their primary focus to deliver customized solutions to address the unique needs of each patient.

Dermaplaning

Dermaplaning is a very effective, safe, physical exfoliation procedure which clears away superfluous, dry skin layers as the initial step in the micropeel. This clinical procedure is undertaken by doctors, or medical skin care specialists who are trained in the use of a modified surgical blade meant especially for this purpose.

Dermaplaning mitigates the appearance of dry fine lines that are visible on aging skin. It also enhances the rate of pharmaceutical and skin care products absorption, which leaves the patient with dazzling skin.

Dermaplaning should not be undertaken on acne-prone skin, active acne, patients with dark, thick hair, or patients experiencing hormonal fluctuations. This procedure is also unsuitable for patients taking specific meds that cause hair growth as a side-effect.

Microneedling

Microneedling therapy involves the use of targeted microneedles to inflict invisible micro-perforations in the skin. These channels promote the production of new collagen to create healthier, more youthful skin.

Microneedling therapy softens the appearance of acne scars, smoothes surface texture, and enables powerful skin care products to penetrate more efficiently into the skin. It is suitable for all skin types and involves minimal downtime. This procedure should ideally be performed as a series of three sessions spaced two weeks apart.

Chemical Peels

Chemical peels are simply chemicals, typically acids, or a blend of acids that are applied in a precise manner to remove the outer skin layers. The depth of the peel depends on the agent itself, the strength of the agent, the application time, pre-treatment regimes, and other aspects.

Various safe options are available to treat different issues. This makes peels a reliable and cost-effective choice for patients with conditions such as acne, photodamage, and dyspigmentation.

Peels are classified as superficial, medium depth, or deep peels. Superficial peels work only on the uppermost skin layers, the epidermis, and the stratum corneum.

Some of the Basics

Glycolic acid is a common agent for superficial peels. It is an alpha hydroxyl acid. The effects of the acid are neutralized after a specific duration, and post-treatment guidelines are given.

The patient can expect very mild redness for a few days as well as dry skin flaking for a little longer. These after-effects can be easily covered with makeup and moisturizers.

Jessner’s solution and salicylic acid are other peel options. Salicylic acid peels are quite lipophilic. Therefore, there are especially helpful in the treatment of active acne vulgaris.

Superficial peels are a general class of peels that are suitable for the treatment of acne, superficial scarring and wrinkles, and dyspigmentation. These peels impact the uppermost skin layers. They are typically applied sequentially over a few months, with an increase in the concentration of the agent with each subsequent peel.

Due to the superficial nature of these medical spa chemical peels, a series of six treatments is often recommended and administered at two to three-week intervals.

Medium depth peels impact the next layer of the skin, namely the dermis. Therefore, these peels have a deeper effect. Trichloroacetic acid (TCA) is used for medium depth to deep peels, depending on the concentration of the acid.

TCA does not require neutralization, unlike glycolic acid. However, its effects include ‘frosting’ the skin, turning it a bit white as the keratin in the skin coagulates.

Another manner of accomplishing a medium depth peel is layering together a combination of various superficial peels, such as TCA and Jessner’s solution. Generally, medium depth peels involve slightly more downtime. The patient can expect redness for seven to ten days with mild discomfort for one to two days.

These peels offer more aggressive action leading to more dramatic outcomes than superficial peels in terms of removing wrinkles, improving skin texture, and helping the skin tone appear more even.

Combination therapy offers beautiful results. At Suncoast Skin Solutions, the skin specialists combine one of the above peels with light or laser-based techniques, such as intense pulsed light (IPL), to promote collagen remodeling and eliminate pigmented lesions, or they may recommend topical prescription drugs such as Retin A or Tazorac to fight wrinkles, or inject dermal fillers and Botox to address deeper wrinkles and lines.

Peels and a quality skin care regimen can accomplish visibly beautiful outcomes without any significant downtime.

Botox

Botox is a revolutionary non-invasive age-erasing treatment that was originally pioneered by dermatologists and perfected over the past decade. It is a purified derivative of botulinum toxin, which blocks the release of a neurotransmitter leading to chemical denervation.

The denervation (paralysis) is localized. The treatment provider will identify the appropriate muscles in the face to inject causing transient reduction of facial wrinkles.

The visible effects of Botox injectables can sustain for three to four months and then wear off. The paralyzed nerve endings gradually start recovering function. This recovery of the inactivated nerve endings causes the loss of effect several months after the procedure.

An understandable common concern for first-time Botox patients is safety. But the amount of Botox that may be lethal is around 3,000 units or 30 bottles of Botox. Only injections of less than 100 units are used for cosmetic purposes. Botox has an unparalleled safety record spanning over ten years of vigorous clinical use.

Botox was originally used to eliminate wrinkles developed through years of muscle use. However, now it is used to prevent lines from developing in the first place.

Dr. Ewanowski is highly experienced in the treatment of the upper and lower third of the face and neck. The treatment of these areas necessitates a significant finesse in technique.

Sunspots

A spot can be known by many names such as sunspot, solar or actinic lentigo, lentigo simplex, freckle, liver spots, or ephelides. Irrespective of the name, these skin blemishes indicate cumulative sun damage. Due to the strength of the sun in the Bay area, these spots are a common occurrence.

According to some studies, the incidence of sunspots is over 90 percent in adults aged 60 years and above! Numerous solar lentigines on the shoulders and upper back area of adults may indicate past severe sunburn and may be used as a marker for the population at high risk for developing skin cancers.

Patients visiting Suncoast Skin Solutions, even for minor or medical spa cosmetic treatment, will be offered a comprehensive skin exam. This is the commitment of Dr. Ewanowski and his team to maintain top-notch standards of patient care.

Dermal Fillers

The face can experience significant volume loss due to aging, which may result in deep wrinkles and furrows. These depressions are perceptible to the human eye due to the light in the ‘valleys’ of the face.

This retained light in the form of linear shows leads us to estimate age accurately. To address this issue and help a patient appear more youthful, these depressed areas must be treated. There are two routes to correction.

Other than invasive surgery, few options existed to the correction of mid and lower face aging. Facelifts and other surgical procedure pull the skin tight, similar to a bunched up bed sheet that can be made flat by stretching the edges.

Filling in the valleys is another way to eliminate wrinkles. The low spots are eliminated by filling them with injectable dermal filler. This is a straightforward, in-office procedure with less downtime and noticeable outcomes.

Collagen is one of the earliest available options and was a viable choice. However, testing for allergy is necessary prior to injecting collagen to ensure the absence of an allergic reaction.

But hyaluronic acid derivatives are recent developments in dermal filling technology. No prior testing is necessary when administering these fillers. Brand-name fillers in the hyaluronic acid family include Juvederm, Restylane, Captique, and Perlane.

There are many new products available today, including Radiesse which comprises microspheres of calcium hydroxyl appetite suspended in an aqueous gel carrier, or Sculptra, which is a poly-L-lactic acid substance necessitating multiple treatments.

Restylane and other HA fillers maintain their shape by using the body’s own moisture. The HA in Restylane is hydrophilic (water-loving), and it soaks up and retains water to create a plumping effect underneath the skin.

A Pleasant Affect

HA slowly degrades slowly over time. This allows each molecule to bind to more water creating the same fullness with less HA. This water binding ability of HA allows it to create sustainable outcomes.

Nasolabial folds and the lip area respond well to treatment with injectable fillers such as Restylane. However, beautiful outcomes depend on the choice of correct filler type, the correct amount of filler, and the timing of the medical spa procedure.

While all options are available at Suncoast Skin Solutions, the expert practitioners will choose a treatment that is most suitable for the specific patient. The practice does not follow a ‘cookie-cutter’ approach and pledges to deliver unique solutions customized to the patient’s needs.
