Important Message About Appointment

The protection of our patients, colleagues, and community is Suncoast’s top priority. Due to the uncertainty and spread of COVID-19, and recommendations given by the CDC (Center for Disease Control and Prevention) we are asking patients to refrain from bringing spouses, other family members, and friends who are not medically or legally required to be at the appointment.

Legal documentation and infectious disease screening will be required in order for anyone other than the patient to be allowed at the visit.

In addition, ALL patients will complete an infectious disease screening form
prior to being allowed back.

If a new patient, please arrive 15 minutes early for paperwork with your insurance card and photo ID.   FACE MASK or FACE COVERING required at the time of the visit.

If you have tested POSITIVE for COVID-19 in the past 30 days please call 844-786-3376 to reschedule.
